Pathways Project to lessen ER wait times

$1.3 million is going into a fund to help seniors live independently, even if help is needed. The North East Specialized Geriatric Services will take the money, and use it to work with community partners. The Pathways Project aims to help seniors have a decline in health get the care they need.

They are going to help fast-track seniors who have had a decline in their help get to a geriatric assessor. From there, they will either be directly admitted into the hospital, or will get home support, which could mean rehab, or enhanced home care. The funding will go into helping seniors with complex medical issues, and who need support. This will help with unnecessary long term care, and hospital admissions.

The project will start in Sudbury and Sault Ste. Marie, but by the second year will be in Temiskaming and Cochrane.
